The RAND Corporation’s 2024 finding that over 80% of AI projects fail to deliver business value is usually interpreted as a technology problem. It is not. The models work. The infrastructure scales. The API calls return. What fails is the organizational layer: the humans who cannot agree on what the AI should do, who owns its outcomes, and how to measure whether it is working.
S&P Global’s 2025 survey — 42% of organizations abandoning most AI initiatives — is the number that should concern you most. These are not experimental side projects. These are funded, staffed, executive-sponsored programs that still collapsed. The common thread is not bad engineering. It is stakeholder misalignment that compounds until the project becomes politically impossible to continue.
This is an opinion piece, informed by what we see in enterprise AI engagements at Clarity. The four failure modes below are not exhaustive, but they account for the vast majority of alignment-driven project failures we encounter.
Failure 1: Conflicting Priorities Across Departments
The sales team wants AI that closes deals faster. The product team wants AI that improves user retention. The risk team wants AI that does not create liability. The finance team wants AI that reduces headcount costs. Each of these is a reasonable goal. Together, they are an impossible specification.
How This Kills Projects
When stakeholders have competing priorities, every design decision becomes a political negotiation. Should the AI prioritize accuracy (risk team) or speed (sales team)? Should it be conservative (compliance) or creative (product)? Each meeting surfaces new constraints from new stakeholders, and the technical team ends up building for the loudest voice in the room rather than a coherent product vision.
The result is an AI system that tries to satisfy everyone and satisfies no one. It is accurate enough to seem promising in demos but too slow for sales workflows. It is fast enough for internal testing but not reliable enough for customer-facing deployment. Six months of development produces a system that each stakeholder considers half-finished.
How to Prevent It
Priority stack-ranking before development starts. Not a list of “nice to haves” and “must haves” — a strict ordered ranking where each stakeholder group sees exactly where their priorities fall relative to others. This creates uncomfortable conversations early, which is the point. Uncomfortable conversations in month one are cheap. Uncomfortable conversations in month eight are expensive.
Single decision-maker with authority. One person must have the authority to break ties between competing stakeholder priorities. Without this, AI projects default to design-by-committee, which is how you get systems that are sophisticated and useless simultaneously.

Failure 2: Unclear Ownership of AI Outcomes
Who owns the AI after it ships? In traditional software, ownership is clear: the product team owns the feature, engineering owns the infrastructure, support owns the tickets. AI blurs these boundaries because AI outcomes span multiple departments.
How This Kills Projects
When the AI makes a bad recommendation, who fixes it? When model performance degrades, who detects it? When a customer complains about an AI-generated response, who responds? In organizations without clear AI ownership, these questions trigger a round of finger-pointing that delays resolution by days or weeks.
The ownership vacuum also kills projects before they launch. Without a clear owner, nobody has the authority to make the final “ship it” decision. The project sits in an extended review cycle where each department raises concerns but none takes responsibility for accepting the remaining risk.
Gartner’s finding that 30% of GenAI projects are abandoned after POC (July 2024) is partly a measurement of the ownership gap. The POC succeeds because a single team owns it. The production deployment fails because no one owns the cross-functional complexity.
How to Prevent It
Define an AI product owner before the first line of code. This person owns the outcomes of the AI system — not just the technical implementation, but the business results. They have authority over the model behavior, the success metrics, and the go/no-go decision for production deployment.
Create an explicit RACI matrix for AI-specific responsibilities. Standard RACI matrices cover software operations. AI systems need additional rows: model monitoring, retraining decisions, bias detection, incident response for AI-specific failures, and escalation paths for edge cases.
Establish clear handoff boundaries. When does the AI team’s responsibility end and the operations team’s begin? When does the product team’s authority override the engineering team’s recommendation? These boundaries must be documented and agreed upon, not assumed.
Failure 3: Success Metric Disagreement
This is the most insidious alignment failure because it often goes undetected until the project is finished. The AI ships. The engineering team celebrates. The business team asks “where are the results?” — and it becomes clear that “results” meant different things to different people.

The engineering team measures model accuracy: 94% on the test set, ship it. The product team measures user engagement: adoption is at 12%, that is low. The executive team measures ROI: we spent $1M+ and revenue has not changed, this failed.
All three assessments can be simultaneously true. The model is accurate. Users are not adopting it. Revenue has not moved. The project is technically successful and a business failure. This outcome is so common that BCG’s 2025 research found 74% of companies struggle to achieve and scale value from AI — despite many of those companies having working models.
The root cause is measuring the wrong things. Model accuracy is a necessary but insufficient condition for business value. User adoption is an intermediate metric that does not guarantee revenue impact. ROI is a lagging indicator that takes months to materialize. If your stakeholders have not agreed on which metrics matter, at which time horizons, with what thresholds — you will build a product that succeeds by some measures and fails by the only one that matters to the person controlling the budget.
How to Prevent It
Three-tier metric framework. Agree on metrics at three levels before development starts:
- Technical metrics (engineering owns): Model accuracy, latency, uptime, error rates. These validate that the system works as designed.
- Adoption metrics (product owns): Active usage, task completion rates, user feedback. These validate that people use the system and find it useful.
- Business metrics (executive owns): Revenue impact, cost reduction, efficiency gains. These validate that the investment was worthwhile.
Each tier has defined thresholds, measurement methods, and review cadences. The key agreement is that technical success does not equal project success — and that business metrics take time to materialize, so the project should not be killed at month three because revenue has not changed yet.

The specific numbers will vary by project. The structure should not.
Failure 4: Change Management Gaps
AI changes how people work. That sentence is obvious and almost universally underestimated. Teams budget for the technology and skip the organizational change required to make the technology useful.

A company deploys an AI system that automates 40% of a team’s manual workflow. The technology works perfectly. But the team was not prepared for the change. They do not trust the AI’s outputs, so they manually verify every result — eliminating the efficiency gains. Or they resent the implication that their work can be automated, and they find reasons to avoid using the system. Or they were never trained on the new workflow, so they use the AI incorrectly and get worse results than the manual process.
The AI becomes shelfware — technically operational, practically unused. The executive sponsor sees low adoption numbers and pulls funding. The project is labeled a failure, even though the technology performed as specified.
How to Prevent It
Involve end users in discovery, not just deployment. The people whose workflows will change should be consulted before the system is designed, not after it is built. Their input shapes the product in ways that improve adoption. Their early involvement builds buy-in that no training program can replicate.
Budget for training as a line item, not an afterthought. Training is not a 30-minute demo at launch. It is ongoing support during the transition period, documentation tailored to specific workflows, and a feedback channel where users can report problems without feeling like they are criticizing the technology.
Measure adoption milestones, not just launch dates. A phased rollout with adoption checkpoints (30-day, 60-day, 90-day) gives the organization time to adjust and gives the project team data to course-correct. Launching to the entire organization on day one maximizes disruption and minimizes learning.
Name the fear explicitly. In many organizations, the unstated concern is that AI will eliminate jobs. If that is a possibility, address it directly. If it is not, say so clearly and mean it. Ambiguity breeds resistance, and resistance kills adoption metrics faster than any technical limitation.
The Alignment Tax
Stakeholder alignment takes time and money. Discovery workshops, priority negotiations, RACI matrices, metric agreements, and change management plans are not free. For most projects, the alignment work adds 2-4 weeks and $15K-$30K to the discovery phase.
That investment looks expensive until you compare it to the alternative: a $1M+ project that joins the 42% of abandoned AI initiatives (S&P Global, 2025) because the CTO wanted accuracy, the VP of Sales wanted speed, and nobody defined who owned the result.
